CRSC and getting wife added for VA disability pay

I am currently in Ecuador. I am retired and get retirement pay. I am also rated by the VA at 30% disabled. I have been recieving disability pay from the VA for about 15 years for diabetes, 20%, and residual, cerebrovascular accident associated with hypertension, 10%. and hypertension, 0%, I have received compensation at the with spouse rate. Since diabetes has been a presumptive agent orange disease since I was first entitled disability I have received CRSC reimbursement from DOD Finance for 20% all along. Now hypertension has been added as an agent orange presumptive disease and I need to apply for CRSC for that 10% as well. I have also never been compensated at the with spouse rate by the VA and I have been marrie to my spouse since 1974. Can you provide guidance in these matters?
