Medical Certificate for permanent residency application

Hello all, what is the best way to get Medical Certificate for permanent residency application. It must be issued by General Clinician authorized by the Ministry of Health?

Is it possible to do all medical examinations/tests and get Medical Certificate at a private clinic and then certify it at the Ministry of Health? Please recommend familiar with this process clinic/general clinician, preferably English speaking, good price/quality/speed ratio. How much it would costs at a private clinic? Or maybe some test must be done or wise to do at a public clinic?

Is International Certificate of Vaccination (Yellow Card) recognized? I will get full travel vaccination at home.
