How to apply for Maltese Passport Through Marriage?

I'm hoping someone might be able to point me in the right direction here about applying for a Maltese passport.

I'm a Canadian citizen (but have my Malta residence permit -which expires Dec 2015 according to the sticker in my passport). My husband and I were married in Malta in 2010- we lived there for three years and then moved to the UK. Now that we've been married for 5 years I'm interested in possibly applying for my Maltese passport. I've been trying to find info online but seems a bit confusing and as usual I don't really find Maltese websites very straight forward lol

Does anyone know which form(s) I would need to complete to apply? Also I have the original Malta ID card. The new rules came into effect after we moved away from Malta so I'm not sure if that is another thing to worry about?

I have no idea where to start and how to apply from the UK, which form I need... so if anyone else has applied for a Malta passport through marriage to a Maltese citizen that could offer advice that would be appreciated  :)

The spouse of a citizen of Malta and as
long  as  you  have  been  married  for  at  least
five    years
and,    on    the    date    of    the
application, you are still married to and living
with that citizen of Malta
