What is the procedure for getting a replacement driver's license? and what is the cost?
Mine is kinda' worn-out, and seems unreadable...and needs to be replaced...

You will need to fill in a form and renewal fee although it may still be valid.
3 passport sized photographs
A letter form your employer
Eye test report (not sure though)
